Register Annotation The purpose of this book is to draw the attention of the scientific community on the implications of persistent angina and to stimulate prospective studies on its prevalence and clinical relevance. A better understanding of the mechanisms precipitating myocardial ischemia after removal of the coronary obstructions would allow the develop- ment of more effective treatment strategies.Ischemia associated with patent coronary arteries may need alternative therapeutic strategies, as compared to ischemia associated with coronary artery obstructions. Pharmacologic or mechanical interventions that increase coronary blood flow and/or reduce myocardial metabolic needs have been proven to be effective when ischemia is secondary to coronary stenosis. In the absence of coronary obstructions, alternative strategies must be identified and tested, starting from agents known to improve microvascular function and agents that can favourably mod late cardiac energy metabolism and improve cardiac efficiency.
